# Human in the loop AI: where the person goes

Human in the loop AI works best when the person confirms at the point where work is recorded, not when they watch the agent. Here is where to put the gate.

Your reviewer has the AI's result on one screen and the source document on the other. Nothing on the first screen says which line to doubt, so they read every line, then read them again. Human in the loop AI is meant to prevent exactly this: a person confirms what the AI produced before it counts. Put the person in the wrong place and the typing simply turns into rereading, and the hours never come back.

## Reading everything twice

The numbers behind that feeling are not small. In a [G2 survey of 1,019 business software buyers](https://company.g2.com/news/2026-ai-agents-report), run in July 2026 and published in October, 31% said staff spend 6 or more hours a week overseeing agent work. 45% said overseeing and correcting agents eats more than a quarter of the gains they expected. Only 7% were comfortable with full autonomy.

Two cautions. It is a software review company's survey and the answers are self reported. And it counts hours spent, not whether those hours were spent in the right place. That second part is the one you can change.

Most coverage reads these numbers as the price of using AI: if you want the speed, accept the babysitting. We read them the other way. Oversight is not the cost. Oversight in the wrong place is.

## Watching an agent costs more than confirming its result

A person who watches an agent sits beside the work. Output appears line by line, and because none of it is marked as shaky, every line gets the same suspicion. The reviewer turns into a second reader, and a second reader of everything is slow by design.

A person at the commit point stands somewhere else: at the moment just before anything is recorded. The AI has read the document and suggested values, and nothing is saved yet. The reviewer looks at each value beside its source and confirms it. Only then does it become part of the record. The job is smaller, and it has an end.

That is how we built the [compliance portal](https://www.mantaq.co/case-studies/compliance-portal) for a behavioral health operator. Licences and training certificates are read when they are uploaded and their values are suggested. Nothing is committed until a person confirms it, and approval stays blocked until they do. In [Stay Funded 360](https://www.mantaq.co/case-studies/stay-funded-360), the system reads the subtotal, tax and fees off a receipt, and an invoice becomes one draft expense per line. Again, nothing is recorded until someone confirms it.

A behavioral health operator and a grant-funded nonprofit have little in common, and in both we put the person in the same spot. The reviewer never has to decide which values deserve suspicion, because every value waits for them, one at a time, next to the thing it came from.

The other term you will meet is human on the loop, where a person supervises a running agent and steps in when something looks wrong. It is closer to watching. It can be right for steps that are low risk and easy to undo. For anything that becomes a record, a payment or a letter someone receives, the gate belongs in front of the record.

## A gate blocks, it does not warn

Placing the person correctly is half of it. The other half is making the gate real. Blocked, not warned.

In Stay Funded 360, a missing receipt stops the month's download and names the record that is missing. An incomplete packet cannot reach the fiduciary, the party that sits between the nonprofit and the funder. Nobody has to remember to check, and nobody has to hunt for the gap, because the system has already said which receipt it is.

A warning does less. It asks the reviewer to notice, and a warning that can be clicked past teaches people to click past it. A block gives them one specific thing to fix. That is also the answer to rubber stamping: a person who has to resolve a named item cannot approve on autopilot, and a person with a general duty to be careful often will.

Some gates stay in front of a person no matter how good the reading gets. On the [claims platform we built for veterans](https://www.mantaq.co/case-studies/va-claims-made-easy), documents are drafted automatically, then reviewed and edited by a VA agent or a doctor before a veteran ever sees them. Reading the records saves hours. A wrong claim costs a veteran their benefit, so the person stays.

## What we let run without a person

A gate on everything would put the reviewer back to rereading. Plenty of steps do not need one, because they move or assemble information without changing what it means.

At [PeopleWorks](https://www.mantaq.co/case-studies/peopleworks), an HR services company, weekly reports used to be put together by hand and now build themselves, and signing follow-ups chase themselves. In Stay Funded 360, once a person has reviewed the month, the system produces the cover sheets, the Excel summary and the merged PDF without anyone touching them. Reminders in the compliance portal run on one fixed schedule for everyone.

The limit is worth saying plainly. Confirming does not fit everything. A step that is low risk and easy to undo can run without a gate, with someone spot checking the output now and then. The line between those steps and the ones that need a gate is a judgment, and it is easy to draw it too cautiously. We are happy to talk through where it falls for a given process.

## Where to put the gate in your own process

Take the process your team reads twice today and ask three things of the step where the AI's output is used:

1. Where does this result first become a record, a payment or a letter that someone receives?
2. What would the reviewer need to see beside the value to judge it in seconds?
3. If a value is missing or wrong, can the system refuse to go on and say which one?

The gate goes at the answer to the first question. The second decides what the reviewer's screen shows. The third decides whether it is a gate or just a warning.

Back at that desk with two screens, the change is small. The first screen now marks the one value that does not match its source, and the approval waits until the reviewer has dealt with it. If one process in your team looks like this, we can map it with you in a [30-min call](https://www.mantaq.co/#contact), and our [services page](https://www.mantaq.co/services) shows how an audit, a build and ongoing support fit together.

## Frequently asked questions

### What is human in the loop AI?

It means a person reviews or confirms what an AI produced before the result is recorded or acted on. The AI does the reading and drafting, and the person keeps the decision.

### What is the difference between human in the loop and human on the loop?

In the loop, a person confirms at a step before work moves on. On the loop, a person supervises a running agent and steps in when something looks wrong. The first holds the work at a gate, and the second watches it go by.

### Does human review cancel the savings from AI?

It can, when the person has to reread everything the AI did. Placed at the point where work is recorded, with each value shown beside its source, the review is a short confirmation and most of the saving stays.

### Which steps should always have a person?

Any step where a wrong result reaches a customer, a record or a payment before anyone can catch it, and any step where someone puts their name to the result. In our veterans' claims system, every drafted document is reviewed by a VA agent or a doctor before the veteran sees it.

### How do you stop reviewers rubber stamping?

Make approval depend on the work. Block until each value is confirmed, show it beside its source, and name whatever is missing, so the reviewer has a specific job instead of a general duty to be careful.
